From quote request to member access.

  1. Submit your group plan request

    Tell us about your organization, who you want to cover, and the number of people you expect to enroll.

  2. Receive your group quote

    We determine the appropriate group pricing tier and provide your monthly rate, minimum enrollment commitment, setup fee, and plan terms.

  3. Select your funding structure

    Choose whether your organization will fully sponsor the membership, share the cost, or collect participant contributions internally.

  4. Complete your agreement and enrollment

    Your organization signs the group agreement, submits the required enrollment information, and pays the applicable setup fee and first month’s membership invoice.

  5. Activate your members

    Each enrolled person receives an individual account and instructions for accessing their rX services.

  6. Give your people a better front door to care

    Members can begin using their available healthcare, prescription, counseling, and veterinary telehealth benefits following activation.

Frequently asked questions.

Is rX health insurance?

No. rX by LVLX is a prepaid healthcare membership and is not health insurance.

It does not replace major medical insurance, hospitalization coverage, emergency services, or other insurance products. It may be offered alongside an existing insurance plan or as an additional healthcare-access benefit.

Does someone need insurance to use rX?

No. An enrolled member is not required to have health insurance to access the services included with the rX membership.

Who can be enrolled through a Group Plan?

Depending on the organization, eligible enrollees may include employees, independent contractors, volunteers, members, agents, representatives, adult students, and other affiliated adults.

Your organization will identify the eligible population during the quote and contracting process.

Does each person receive an individual account?

Yes. Every enrolled person receives an individual login and accesses services through their own rX membership. Accounts should not be shared.

Can spouses and children be included under an employee’s Group Plan membership?

No. rX Group Plans are individual-only memberships and do not include spouse, family, household, or dependent coverage. Minors may not enroll as primary members under a Group Plan.

Separate individual or family rX membership options may be purchased outside the organization’s Group Plan.

Can our organization offer rX even if we already provide health insurance?

Yes. rX can complement an existing benefits package by making common, everyday healthcare needs easier to access.

It may be particularly valuable for people dealing with high deductibles, copays, limited appointment availability, or difficulty accessing care during traditional office hours.

Can we offer the plan to part-time employees or contractors?

Yes. Organizations may establish eligibility for part-time employees, independent contractors, volunteers, members, or other eligible adults, subject to the Group Plan agreement.

Does the organization have to pay the entire cost?

No. Your organization may:

  • Pay the full membership amount
  • Subsidize a portion of the cost
  • Collect the full cost from participating individuals internally

However, the organization signs the agreement, pays LVLX directly, and remains responsible for the contracted monthly amount.

Are all doctor visits included without a copay?

Urgent care, primary care, and other specifically included services are available without a copay, subject to plan terms, clinical appropriateness, and responsible use.

Certain specialty services, tests, medications, referrals, or outside care may involve additional costs.

Are all prescriptions free?

More than 800 medications are available at $0 when the medication is prescribed, clinically appropriate, and included on the current rX formulary.

Medications outside the formulary, brand-name medications, specialty medications, and certain pharmacy services may involve additional costs.

Is counseling included?

Yes. Members receive 24/7 access to counseling and crisis management.

Specialty mental and behavioral health consultations or ongoing specialty treatment may carry additional member fees.

Is emergency care included?

No. rX is not an emergency response service.

Anyone experiencing a medical or psychiatric emergency should call 911 or go to the nearest emergency department.

How quickly can a Group Plan be activated?

Activation depends on completion of the agreement, receipt of enrollment information, payment of the applicable setup fee, and payment of the first month’s invoice.

The Group Plan team will provide the anticipated activation timeline with your quote and onboarding instructions.

Is there a setup fee?

Yes. A one-time, nonrefundable setup fee applies based on the approved pricing tier. Setup fees range from $99 to $499.

How long is the Group Plan agreement?

Group Plan agreements are for 12 months and automatically renew for an additional 12-month term unless written notice of nonrenewal is provided at least 90 days before the renewal date.

What happens if fewer people participate than expected?

The organization remains financially responsible for the minimum enrollment level and monthly billing amount associated with its contracted tier throughout the agreement term.

Your organization should provide a realistic enrollment estimate when requesting its quote.
